**NEW** Ground Industrial vs Aviation Ground Ops
Added by Ron DuMont on Dec 22 2009 11:29

It has been noted that several Ground and Industrial/Industrial and Ground and Industrial/Miscellaneous mishaps have been released categorized incorrectly IAW AFI 91-204, Chapter 1, paragraph 1.8.1.3.3.

* If a mishap involves and aircraft with NO-Intent-for-Flight, that is not being handled as a commodity or cargo, it will be categorized as an Aviation mishap (Aviation Ground Ops).

Generally, mishaps involving maintenance will use AFMAN 91-224 (Grnd) procedures
and mishaps involving operations (ex: taxi mishap) will use AFMAN 91-223 (Flt) procedures.

NOTE: The Convening Authority will determine which procedures are more appropriate.

NOTE: AFMAN 91-224 investigations will be cross-categorized as Ground and Industrial, Industrial mishaps.

* Prior released mishaps that fit this category have been re-opened.

Change the report's category to "Aircraft Ground Operations" as follows.
• Change the CATEGORY field to "Aviation"
• Change the Subcategory Tier 1 field to "Aircraft Ground Operations"
• Change the 1st Cross Category field to "Ground and Industrial"
• Change the 1st Cross-Subcategory field to "Industrial or Miscellaneous (depending on the mishap)
• And change the 2nd Cross-Category field to "None"

All fields/screens will need to be re-reviewed for additional mandatory information that may have changed due to changing the above fields/screens.

Verify that the Narrative and Findings match the entry order of the objects.

After running the error check, because the category is now AGO:
• The aircraft (and applicable screens) will be mandatory as Object 1
• The aircraft component (and applicable screens) will be mandatory as Object 2

NOTE: if the Object 2 is an "Engine", the aircraft engine component (and all applicable screens) will be mandatory as Object 3. (If the component is repairable or destroyed, the cost will go on the component; list the aircraft as Not Damaged and there will be no cost associated. Most engine serial numbers are six-digits. Ensure you enter the full tail number (ex: 78-11111)).

After the corrections are made, a Final (Supplemental) will need to be released.

Any released reports received from this day on that fit this category and are reported as Ground and Industrial/Industrial or Ground and Industrial/Miscellaneous will be rejected. As in all rejections, after corrections are made, release the Final (Supplemental).
